After Hours Call Charge

Dear Valued Patients of Southlake Family Medicine,

As you are aware, the physicians of Southlake Family Medicine provide 24 hour call coverage for medical emergencies. This is a service that we are glad to provide and that we feel is an important resource for you and your family.

Due to the costs involved with managing after-hours calls, it has become necessary to institute a nominal charge for these after-hours phone calls. Beginning July 1, 2010, calls after business hours may incur a $15 charge. This will not be billed to insurance, but will be posted to your account at the office. This charge will not apply to calls due to errors on our part, such as prescriptions that did not transmit to the pharmacy.

There you will find a library of information about common Pediatric problems, as well as updates regarding office policies and other news related to our office. Baylor Family Medicine at Riverside has an after-hours Family Medicine clinic which our patients can utilize for urgent visits. The address is 2740 N. Highway 360, suite 100, Grand Prairie 75050.

New Extended Acute Care Hours

We are pleased to offer After-Hours Clinic for Acute Care of established patients of Southlake Family Medicine.

Monday through Thursday 5-8 pm. Appointments for evening hours can be made "same day" by calling our office after 1 pm. Walk-in patients will be seen as availability will allow. Ages seen are 5 years and older. There is an additional convenience fee of $20.

What is considered acute care?

• Cough, Colds, Fever & Flu
• Migraines & Headaches
• Eye & Ear Infections
• Skin Conditions i.e. Rashes
• Insect Bites
• Minor Drug Reactions
• Minor sprains
• Urinary Tract Infections

What is not considered acute care?

Emergency care, which includes:
• Fractures and Major Sprains
• Chest Pains / Breathing Complications
• Head Injuries
• Lacerations
• Severe Allergic or Drug Reactions
• Other Critical Care / Life-Threatening Situations
For emergency care, call 911 or visit the nearest Emergency Room.

Routine care, which includes:
• Ear Flushing
• No Prescription Refills
• Camp, Pre-Op or Immigration Examinations
• Routine Blood-Work & Check-Ups or Physicials
• Doctors Forms i.e. School or Back-To-Work Notes
