FluSOUTHLAKE FAMILY MEDICINE DID NOT RECEIVE ANY OF THE RECALLED H1N1 VACCINETypes of Vaccine AvailableSeasonal FluPreservative Free Inactivated Vaccine Intended for children 6 months through 35 months Seasonal Flu Vaccine Intended for all adultsH1N1 Because this vaccine is being supplied by the US government, we are required to adhere strictly to the government’s vaccine administration guidelines. Therefore, we will only administer the Flumist vaccine to ESTABLISHED Southlake Family Medicine patients who fall into the following categories:Influenza A Monovalent Vaccine (Injection) Healthcare workersAll children 6 months and older All adults Pregnant women *This vaccine is not thimerosal freeLive Attenuated Intransal Vaccine (Flumist) Healthy Children ages 2 yrs thru 18 yrs – No underlying medical conditions. Persons from age 19 yrs thru 49 years -- who themselves are healthy & who are caring for or living with a child or children under the age of 6 monthsPlease note this form of the vaccine (the flumist) will NOT be given to anyone in the following categories:Anyone with a history of asthma or other chronic medical conditionsPregnant womenAnyone over the age of 49 yrs *Anyone who has had a positive flu test in July 2009, August 2009 or September 2009, is already immune to H1N1 and will not need the vaccine. Please take the time to fill out our H1N1 Vaccine Form located on our Forms page.We will be collecting a $25 administration fee due at the time the vaccine is given. *This service is available to all established patients of Southlake Family Medicine; please help us get the word out!!!*Please call to schedule your flu shot!For all types of flu vaccines if a child is under the age of 9 years, then the CDC recommends that you get a second injection 4 weeks later; there is a $25.00 charge for each H1N1 injection.
